近日,诺华公布了ALIGN III期研究的2.5年终期数据,在IgA肾病(IgAN)成人患者中,与安慰剂相比,盐酸阿曲生坦片可减缓肾功能的下降速率。该结果已发表于《柳叶刀》杂志,并在欧洲肾脏协会(ERA)大会上公布。

估算肾小球滤过率(eGFR)较基线的变化结果显示:盐酸阿曲生坦片治疗具有优势,并且在整个治疗期间,使尿蛋白持续降低。无论是采用不同的肾功能评估指标,还是在同时接受钠-葡萄糖协同转运蛋白2(SGLT2)抑制剂治疗的患者中,其获益均保持一致1,2。

“这些结果有力证明了在两年多治疗期间,肾功能的下降可实现具有临床意义的减缓,也进一步印证了此前关于蛋白尿减少的分析结果。”ALIGN研究研究者兼指导委员会成员、斯坦福大学医学中心肾小球疾病中心主任、肾脏病学教授、医学博士、美国内科医师学会会员Richard Lafayette表示,“这些结果凸显了高选择性内皮素A受体拮抗剂在IgAN不断发展的治疗策略中的作用。”

安全性与既往研究一致,不良事件与安慰剂相似,且未观察到新的安全性信号1-4。

盐酸阿曲生坦片已于2025年在美国和中国获得加速批准,用于降低成人IgAN患者的蛋白尿5,6。诺华计划以这些数据支持其于2026年申请常规批准。

关于IgAN

IgAN是一种进行性的自身免疫性肾脏疾病,全球每年每百万人中约有25人新确诊7,8。IgAN具有较高致残性,可导致肾脏微小滤过结构发生炎症、蛋白尿升高,以及eGFR逐步下降9。持续性蛋白尿的患者中,多达50%会在确诊后10至20年内进展为肾衰竭,通常需要透析或肾移植作为长期疾病管理的一部分8-13。
